บทคัดย่อ (ภาษาไทย)

การวิจัยนี้มีวัตถุประสงค์เพื่อ 1) วิเคราะห์สภาพปัจจุบัน ปัญหา และข้อเสนอแนะในการ ดูแลรักษาผู้ป่วยมะเร็งแบบประคับประคอง 2) ศึกษาประสิทธิผลการดูแลรักษาผู้ป่วยมะเร็งแบบ ประคับประคองที่คาดหวังและที่เกิดขึ้นจริง 3) ศึกษาคุณลักษณะขององค์กรที่ส่งผลต่อประสิทธิผลการ ดูแลรักษา 4) ศึกษาคุณลักษณะของการดูแลรักษาผู้ป่วยและความพร้อมของปัจจัยการจัดการที่ส่งผล ต่อประสิทธิผลการดูแลรักษา และ 5) วิเคราะห์และเสนอแนวทางการจัดการเพื่อเพิ่มประสิทธิผลของ การดูแลรักษา เก็บรวบรวมข้อมูลเชิงปริมาณจาก แพทย์ พยาบาล และบุคลากรที่รับผิดชอบงานการ ดูแลผู้ป่วยมะเร็งแบบประคับประคอง จํานวน 558 คน และเก็บข้อมูลเชิงคุณภาพจากผู้ให้ข้อมูลสําคัญ จํานวน 20 คน สถิติและวิธีการที่ใช้ในการวิเคราะห์ข้อมูล ได้แก่ สถิติบรรยาย การวิเคราะห์ความ แปรปรวนทางเดียว และการวิเคราะห์การถดถอยเชิงพหุคูณ ผลการวิจัยพบว่า 1) สภาพการดูแลผู้ป่วยมะเร็งแบบประคับประคองโดยรวม ด้านนโยบาย มีความชัดเจนและมีงบประมาณเพียงพอ แต่ยังขาดหน่วยงานเฉพาะกิจที่รับผิดชอบโดยตรง 2) ความ จําเป็นในการปรับปรุงประสิทธิผลการดูแลรักษาตามความเร่งด่วน คือ ด้านการจัดระบบการดูแลรักษา ด้านผลลัพธ์การดูแลรักษา และด้านกระบวนการดูแลรักษา 3) คุณลักษณะขององค์กรที่แตกต่างกัน มีผลต่อประสิทธิผลการดูแลรักษา อย่างไม่มีนัยสําคัญทางสถิติ 4) คุณลักษณะของการดูแลรักษา ผู้ป่วยและความพร้อมของปัจจัยการจัดการส่งผลต่อประสิทธิผลการดูแลรักษาในภาพรวม เรียง ตามลําดับอิทธิพล ได้แก่ ทัศนคติ พฤติกรรม ความพร้อมด้านบุคลากร ด้านวิธีการปฏิบัติงาน และด้าน เครื่องมืออุปกรณ์ทางการแพทย์ และ 5) แนวทางการจัดการเพื่อเพิ่มประสิทธิผลของการดูแลรักษา คือ (1) จัดตั้งหน่วยงานที่รับผิดชอบการดูแลรักษาผู้ป่วยมะเร็งแบบประคับประคอง (2) สร้างกระบวนการ ทํางานเป็นทีมของบุคลากรผู้ให้การดูแลรักษา (3) สร้างเสริมความรู้ความเข้าใจเกี่ยวกับการดูแลรักษา ผู้ป่วยมะเร็งแบบประคับประคองให้แก่ผู้เกี่ยวข้อง และ (4) สร้างระบบและกลไกในการดูแลคุณภาพ ชีวิตของผู้เกี่ยวข้องในกระบวนดูแลรักษา

Abstract (English)

This research aimed to 1) analyze the current situation, problems, and recommendation for palliative care for cancer patients, 2) study the expected and actual effectiveness of the method mentioned above, 3) study the organizational characteristics affecting the effectiveness of care, 4) study the characteristics of care and readiness of management factors affecting the effectiveness of care, and 5) analyze and propose management guidelines to increase the effectiveness of care. Quantitative data were collected from 558 doctors, nurses, and personnel responsible for the palliative care for cancer patients, and qualitative data were collected from 20 key informants. The content analysis, descriptive statistics, analysis of variance and multiple regression analysis were conducted for data analysis. The research findings indicated that: 1) The overall state of palliative care for cancer patients, in terms of policy is clear and has sufficient budget, but there is still a lack of a specific agency directly responsible for it. 2) The need to improve the effectiveness of care according to the urgency, namely, the organization of care system, the outcome of care, and the process of care. 3) Different organizational characteristics affect the effectiveness of care. 4) The characteristics of patient care and readiness of management factors affected the overall care effectiveness, ranked in order of influence as follows: attitude, behavior, readiness, work methods, and medical equipment. 5) The 4 guidelines for the management to increase the effectiveness of care were (1) establishing specialized unit for the cancer patients palliative care, (2) creating the team work process for caregivers, (3) enhancing knowledge and understanding palliative care for cancer patients for those involved, and (4) creating a system and mechanism to care for the quality of life of those involved in the care process.
