บทคัดย่อ (ภาษาไทย)

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาสภาพปัจจุบัน ปัญหา อุปสรรค ปัจจัยที่ส่งผลต่อสมรรถนะของนักกายภาพบำบัดในผู้สูงอายุ สมรรถนะที่เป็นจริงกับสมรรถนะที่คาดหวังของนักกายภาพบำบัด เปรียบเทียบคุณลักษณะส่วนบุคคลที่มีผลต่อสมรรถนะของนักกายภาพบำบัด ศึกษาแรงจูงใจและวิธีการพัฒนานักกายภาพบำบัดที่ส่งผลต่อสมรรถนะของนักกายภาพบำบัด และนำเสนอแนวทางการพัฒนาสมรรถนะของนักกายภาพบำบัดในผู้สูงอายุ เป็นการวิจัยแบบผสมผสานโดยใช้ข้อมูลจากการตอบแบบสอบถามของนักกายภาพบำบัด จำนวน 410 คน สุ่มตัวอย่างโดยใช้หลักความน่าจะเป็นด้วยวิธีการสุ่มแบบหลายชั้นตอน และข้อมูลเชิงคุณภาพจากการสัมภาษณ์เชิงลึกผู้เชี่ยวชาญ ผู้ป่วยสูงอายุ และญาติของผู้ป่วยสูงายุกลุ่มละ 17 คน สถิติที่ใช้ในการวิเคราะห์ คือ ค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน การวิเคราะห์ PNI การวิเคราะห์ความแปรปรวนทางเดียว การวิเคราะห์ถดถอย เชิงพหุคูณ และการวิเคราะห์เนื้อหา ผลการวิจัยพบว่า ปัจจุบันวิชาชีพกายภาพบำบัดเป็นที่รู้จักมากขึ้น นักกายภาพบำบัดไม่เพียงพอต่อภาระงาน มีช่องว่างระหว่างสมรรถนะที่เป็นจริงและที่คาดหวังที่ต้องพัฒนา ประสบการณ์และสถานที่ทำงานส่งผลต่อสมรรถนะของนักกายภาพบำบัด แรงจูงใจที่ส่งผลต่อสมรรถนะของนักกายภาพบำบัดอย่างมีนัยสำคัญทางสถิติ ได้แก่ ความสำเร็จในงาน การยอมรับในอาชีพ ลักษณะงานที่ทำ ค่าตอบแทน ความสัมพันธ์ระหว่างบุคคล นโยบายและการบริหารงาน วิธีการพัฒนานักกายภาพบำบัดที่ส่งผลต่อสมรรถนะของนักกายภาพบำบัดอย่างมีนัยสำคัญทางสถิติ ได้แก่ การลงมือปฏิบัติ การทดลองเรียนรู้จากสถานการณ์จริง และการได้รับข้อมูลจากหัวหน้ารุ่นพี่ ผู้วิจัยนำเสนอ 8 แนวทางเพื่อพัฒนาสมรรถนะของนักกายภาพบำบัดในผู้สูงอายุ ดังนี้ 1) การพัฒนาอบรม 2) พัฒนาสมรรถนะให้สอดคล้องกับความต้องการของผู้ป่วยสูงอายุ 3) สร้างช่องทางในการเพิ่มพูนทักษะของนักกายภาพบำบัด 4) พัฒนาผู้ช่วยเหลือนักกายภาพบำบัดให้มีความรู้และทักษะที่จำเป็น 5) ปรับปรุงหลักสูตรการเรียนการสอน 6) สนับสนุนการพัฒนาตนเองให้เป็นผู้เชี่ยวชาญ 7) กำหนดมาตรการควบคุมคุณภาพ และ 8) สร้างเครือข่ายนักกายภาพบำบัด

Abstract (English)

This research aimed to examine the current situation, problems, obstacles, and factors influencing the competency of physical therapists in the elderly, to compare actual competencies with expected competencies, to investigate motivation and development that affect their competencies, and to present guidelines for enhancing their competencies. This mixed-methods research utilized data from questionnaires completed by 410 physical therapists. The samples were selected using a multistage random sampling method, and qualitative data were collected through in-depth interviews with experts, elderly patients, and their relatives, with 17 participants in each group. The statistics used for analysis included frequency, percentage, mean, standard deviation, PNI, One-way ANOVA, multiple regression analysis (MRA), and content analysis. The research findings reveal that the physical therapy profession is gaining increased recognition, yet physical therapists are facing inadequacies in handling their workloads. There are gaps between the actual and the expected competencies that need development. The experiences and workplace conditions significantly impact the competency. The motivators influencing the competency include job success, professional acceptance, job characteristics, compensation, interpersonal relationships, policies, and management. Furthermore, developmental methods that impact the competency include hands-on practice, experiential learning from senior colleagues. The researchers propose 8 guidelines for physical therapists competency development: 1. Develop training programs; 2. Develop competencies aligned with the needs of elderly patients; 3. Create avenues to enhance their skills; 4. Develop support staff with necessary knowledge and skills; 5. Improve instruction; 6. Support self-development to become experts; 7. Establish quality control measures; and 8. Create a network for physical therapists.
